What is the Virtual Ward?
The Virtual Ward is a new model of care, designed to support patients who are at high risk of readmission to hospital and their family doctors for a few weeks after hospital discharge. The virtual ward takes the best elements of hospital care (hence "Ward") and applies them to patients living in their own homes. (hence "Virtual")
Who are the members of the Virtual Ward Care Team?
The Virtual Ward Team consists of a Doctor, a Pharmacist, two Care Coordinators, a Team Assistant and (on a part time basis) a hospital based Nurse Clinician and community Nurse Practitioner.

Who does the Virtual Ward care for?
The process for ‘admission’ to the Virtual Ward begins in the hospital and will involve communication between the discharging hospital, the Virtual Ward team and the patient. Patients ‘admitted’ to the Virtual Ward must be referred by the admitting hospital prior to discharge and meet specific criteria, which includes the use of a scoring tool developed that identifies patients at high risk of re-admission.
General internal medicine patients at St Michael's Hospital, Toronto General Hospital, Sunnybrook Health Science Centre and Toronto Western Hospital are currently being admitted into the Virtual Ward.

The Virtual Ward is located at Women’s College Hospital, 76 Grenville Street, on the 6th Floor, East wing. Should you have an appointment with the Virtual Ward, when you arrive on the 6th Floor, inquire at General Reception for directions.
Should you have any questions regarding your appointment staff will be available to answer your questions Monday to Friday 8:30 - 4:30 on the Virtual Ward at 416-323-6363.
For
patients of the Virtual Ward, after hours this phone number will be answered by
the Women’s College Hospital switchboard who will redirect your call to the
Virtual Ward team.
